We are here to help you while the person you care for is in hospital. We can help you to exercise your rights during their stay, and throughout the discharge process.
Who can use this service?
This service currently is designed for unpaid carers who live or provide care in the Borough of Lambeth.
We offer support to unpaid carers, whilst the person they care for is in hospital and following their discharge from hospital. We are here to help carers navigate what can often be a difficult and stressful time, to ensure they are connected to essential information and support for their caring role.
How can we help?
Our team can support you in a number of ways:

Help you think through what help may need when your cared for person leaves hospital.
