Who can use this service?

This service currently operates in specific wards at St Thomas' Hospital and King's College  Hospital. It is designed for Unpaid Carers who live or provide care in the Borough of Lambeth.

​This service is for established Carers, as well as those who will be taking on caring responsibilities following discharge. 

The wards receiving support are:

  • the Byron, Donne or Marjorie Warren wards at King’s College Hospital, Denmark Hill.

  • the Mark, Alexandra, Anne or Henry wards at St Thomas’ Hospital, Westminster Bridge Road.

If you are a Lambeth-based Carer who takes care of somebody on a different ward or different hospital, please contact our Adult Carers Service  and/or fill in our registration form, to learn about available support.

How can we help?

Our Hospital Carers' Lead can support you in a number of ways:

  • Be a listening ear.

  • Help you think through what help may need when your cared for person leaves hospital.

  • Come with you to discharge planning meetings.

  • Providing relevant information for carers and signposting to support.

What is the Carers and Hospital Discharge Partnership?

Age UK Lambeth and Carers’ Hub Lambeth are working in partnership to offer support to unpaid carers, whilst the person they care for is in hospital and following their discharge from hospital. We are here to help carers navigate what can often be a difficult and stressful time, to ensure they are connected to essential information and support for their caring role.


As well as our In-Hospital Carers Support, Unpaid Carers who live or care for somebody in Lambeth can access:

  • The Discharge Helpline - Age UK Lambeth is delivering a helpline that is available Monday to Friday between 9.30am-4.30pm (excluding bank holidays). They are on hand to provide information and advice around what support is available to carers whilst the person they care for is in hospital, to aid preparation for discharge, and to access support following a stay in hospital.

 

  • Welcome Home Scheme - Age UK Lambeth is providing support to carers and those that they care for who've recently been discharged from hospital, for up to four weeks. Their Coordinator can provide information, signposting, support around housing or finances, helping someone become more active, and linking them to new activities, local clubs and local services.
